The short answer is: yes, cocktails can absolutely be low-sugar or even sugar-free. The longer answer is that it depends on the cocktail, the spirit, and the mixer.

What Adds Sugar to a Cocktail?

The main sources of sweetness in a cocktail are: syrups (simple syrup, flavoured syrups), liqueurs (Amaretto, triple sec, and similar), fruit juices (particularly orange and pineapple), and sweet mixers (tonic has some sugar; ginger beer more). Spirits themselves — gin, vodka, whisky — are not sweet. The sweetness is added deliberately.

Knowing where the sugar comes from makes it much easier to ask for less. 'No syrup', 'less sweetener', or 'drier' are all clear requests that any bar can accommodate.

The Driest Options

Dry Martini: gin plus minimal vermouth — essentially no added sugar, just the spirit and a whisper of herbal complexity. Whisky Highball: whisky and soda, clean and dry by default. The Negroni: gin, bitter, and vermouth — the bitterness balances any sweetness, making it not a sweet drink despite the vermouth. Campari Spritz: drier than Aperol, with a more prominent bitter edge.

'Drier' means less sweet, less syrup. 'Less sweet' means reduce the sweetening element. 'Dry spirit forward' means let the spirit speak without sweetening it.
